چکیده

Introduction The presence of ischemia-induced microvascular obstruction (MVO) despite successful coronary revascularization, has been associated with poor functional recovery and adverse left ventricular remodeling after acute myocardial infarction (AMI). Additionally, the extent of the infarct gray zone is a strong independent predictor of post-AMI mortality. However, the evolution of gray zone after AMI has not been investigated and its relationship with MVO in terms of risk stratification is unknown.

Temporal changes of microvascular obstruction and infarct border zone after acute myocardial infarction assessed by contrast enhanced magnetic resonance imaging

Background Besides total scar size, scar characterization parameters are emerging as additional prognostic factors after acute myocardial infarction. Of these, microvascular obstruction (MVO) and infarct border zone are the most promising ones. Nevertheless, a MRI scan performed few days after infarction assesses a rapidly changing myocardium. In particular the infarct border zone that is belie...

Microvascular obstruction by delayed contrast-enhanced MRI remains a strong predictor of left ventricular remodeling even after successful reperfusion in acute myocardial infarction

Background Microvascular obstruction (MVO) after coronary reperfusion for acute myocardial infarction (AMI) implies advanced myocardial damage and is recognized as a negative prognostic value. Delayed contrast-enhanced MRI is the preferred MR imaging technique for evaluation of MVO. In this study, we assessed the influence of MVO on post-infarction left ventricular (LV) remodeling and dysfunction.

Microvascular obstruction is associated with greater infarct heterogeneity

Methods 21 patients post primary percutaneous coronary intervention for acute ST elevation myocardial infarction were prospectively enrolled and each had 3 MRI scans on a 1.5T GE Signa Excite scanner at 48 hours, 3 weeks and 6 months post infarction. A T1-weighted IR-GRE sequence was used for late gadolinium enhancement assessment of infarcted myocardium. Core infarct size (CIS) and gray zone (...

Myocardial Salvage is Reduced in Primary PCI-Treated STEMI Patients with Microvascular Obstruction, Demonstrated by Early and Late CMR

OBJECTIVES This study evaluates the association between microvascular obstruction and myocardial salvage, determined by cardiac magnetic resonance performed both in the acute stage of myocardial infarction and after 4 months. METHODS In patients with acute ST-elevation myocardial infarction treated by primary percutaneous coronary intervention, myocardial salvage, infarct size, left ventricul...

Post-reperfusion lymphopenia and microvascular obstruction in ST-segment elevation acute myocardial infarction.

INTRODUCTION AND OBJECTIVES The presence of microvascular obstruction after ST-segment elevation acute myocardial infarction is associated with a poor outcome. The pathophysiology of this process has not been fully defined. The aim of this study was to investigate the relationship between post-reperfusion lymphopenia and microvascular obstruction. METHODS This prospective study involved 212 p...
